At Piedmont Urgent Care, our providers will check vitals, vision, joints, and flexibility, as well as review a patient’s medical history. A short fitness assessment may also be conducted to identify any possible limitations on physical activity, ensuring they’re ready for both sports and camp activities.

Sports & Camp Physicals
- No appointment needed, walk-ins are always welcome.
- Bring any necessary forms required by your school, camp or sports team and glasses or contacts if you wear them.
- Forms must be completed in their entirety prior to patient registration.
- Minors require a parent or legal guardian present at time of visit. If the athlete or camper is under the age of 18 & does not have a parent or legal guardian present, Piedmont Urgent Care will not treat the patient.
- Any additional lab tests or services will result in additional charges.
*Please note that we do not offer “certificates of immunization” (these copies should be obtained from the administering healthcare provider).
